Intake
A member of our team will reach out to begin the enrollment process, discuss patient needs, answer any questions and explain every step, from start to finish, to put everyone at ease.
Evaluation
Intake coordinators obtain physician referrals and help with insurance paperwork and forms, ensuring everything is submitted correctly. Once insurance authorization is received, a coordinator will reach out to discuss the particulars.
Care
One of our RNs will do a home visit, devise a customized care plan and oversee its implementation. A caregiver will be assigned and works closely with the RN to implement the plan with accuracy.
