CDPAP

Choose a family member or friend to care for you. Medicaid recipients can choose their own caregiver through New York’s Consumer Directed Personal Assistance Program (CDPAP). Amazing Home Care is an experienced fiscal intermediary (FI) and provides the support needed to get a caregiver through CDPAP.

Waiver
Programs

Waiver programs provide services to those who would otherwise be medically eligible for placement in a hospital or nursing facility for an extended period of time if such programs were unavailable. Patients receive case management and may receive other services based on assessment and need.

NHTD

The Nursing Home Transition and Diversion (NHTD) program is an alternative to a nursing home. To receive NHTD, patients must be eligible for Medicaid, require nursing home level care, be between 18 and 64 with a physical disability, or be age 65 or older and can live safely in the community with services.

TBI

Traumatic Brain Injury (TBI) is a Medicaid waiver program that allows individuals who have suffered a traumatic brain injury to live within the community setting of their choice. Services include independent living skills training, day programs, respite care, assistive technology, service coordination and more.
